Making an AppointmentWeekly office hours are Monday through Thursday, from 9:00 AM to 8:00 PM (Mountain Time). Requests for appointments may be made by phone at (719) 380-9032. If you get my voice mail, please leave a message. My office is located at 4865 Old Farm Circle West in Colorado Springs, CO 80917. FeesThe counseling fee is $100.00 for a 50-minute session. Payment is expected at the end of each session, and may be made by cash or check written to Trudy Colflesh. InsuranceIf your insurance covers a Licensed Professional Counselor (L.P.C.), and you are allowed to go out of network, you may receive reimbursement. You, the client, pay the fee upon services rendered and submit claims directly to your insurance company. I will give you a statement with fees and diagnosis (required for insurance) at the end of each month, which is then submitted with the claim. (See Tips on Talking to Health Insurance Companies.) |
